Change of Personal Information Forms
Do NOT use these forms for enrollment purposes. These forms are to be used to Change Personal Information ONLY, such as change of address or phone number. All three forms must be completed. (TRMC Change of Personal Information, Guardian Dental Form and Employee Benefits Form)

Direct Deposit Enrollment/Cancellation Form

Implementation takes two weeks.
If you chose to change banks, please give TRH two weeks notice to process changes after new account has been open.
Do NOT close your bank account before making direct deposit changes with the Accounting Payroll Department.

Sick Leave Buy Back Form
If you have over 288 hours of accrued sick time, then you may sell back any sick time accrued above 288 hours at ½ of your base rate, but you must carry a minimum balance of 288 hours of accrued sick time.
